What information is in the boxes on the 1098-T form?


  • Box 1: Total amount of payments received, minus refunds, for tuition during the reporting calendar year for credit and PSAV classes.
  • Box 2: This box should be blank as MDC reports on paid amounts only.
  • Box 3: This box should be blank as MDC has not changed the reporting method.
  • Box 4: Adjustments to payment, tuition, and fees reported on a prior year 1098-T form due to changes within the reporting year.
  • Box 5: Total amount of scholarships or grants paid during the reporting calendar year. This can include third parties, tuition waivers, Florida Prepaid, but excludes loans.
  • Box 6: Adjustments to scholarships or grants reported on a prior year 1098-T form.
  • Box 7: Checked if the amount in box 1 includes payments for classes for the academic period between January and March of the upcoming year. As an example, Spring 2019 tuition paid during the calendar year 2018.
  • Box 8: Checked if the student was registered for at least half-time or the equivalent of six credit hours for at least one term within the reporting calendar year.
  • Box 9: This box should be blank as MDC does not offer graduate level degrees.
